- What do I need to bring to my first appointment?
You will need your insurance card, a referral from your doctor (if we do not already have a prior authorization) and comfortable clothing. It would also be helpful to bring shoes you wear for exercising or walking. If you are coming from work, feel free to bring a change of clothes and use one of our private treatment rooms.
- What should I expect for my first appointment?
The first time you visit, we will conduct a complete evaluation and thoroughly explain what is causing your limitations and how therapy, intervention will assist. We will discuss the findings with you and tailor a program to meet your specific restorative goals. Specific and realistic objectives are integral to the physical therapy process and identifying those objectives should be a collaborative effort.
- How long are initial evaluations and follow up appointments?
For your first visit, expect to be at the clinic between 40-75 minutes. You will also need to arrive at least 30 minutes early in order to fill out the necessary paperwork, so the therapist can spend the full time scheduled with you. Each subsequent visit will last between 30-60 minutes, depending on the therapist's treatment plan.
- What will happen during my subsequent appointments?
Each subsequent visit, a licensed physical therapist – through personal attention – will gain a thorough understanding of your functional limitations and plan an individualized rehabilitation program. Our programs are customized to meet your needs, and we always work in conjunction and communicate with your prescribing physician.
- Is parking available on-site?
Parking is available in the lot behind the building. When you enter, inform the security guard you are attending a therapy appointment, and you will be directed to a designated Lakeside Community Healthcare Physical and Sports Therapy parking space.
- Will I owe any money at the time of my visit?
Yes, you will be responsible for paying any co-pays due at the time of your visit. We will bill your health plan for all services and will bill your directly for any deductibles or uncovered services.
- How do I cancel or make a change to a scheduled appointment?
We require 24 hours notice to cancel or change an appointment. If changes need to be made after hours, please leave a message for the staff to receive in the morning.
