Lakeside Community Healthcare’s origins date back to 1986, with the formation of Lakeside Medical Group, an independent practice association (IPA), which is a group of independent physicians allied to provide services to managed care organizations. Over the years, the company has grown in scope and size, transforming from a small IPA to one of Southern California’s leading, comprehensive healthcare providers, serving patients with a range of insurance plans, from HMOs to PPOs and more. Lakeside Community Healthcare continues to evolve, as it develops new and even better ways of serving the healthcare needs of the communities it serves.

Lakeside Health Services, Inc., a management services organization (MSO), established to provide management services for independent practice associations, is formed. |

Lakeside Medical Associates, Inc., a multi-specialty medical group, whose physicians are employed by Lakeside Community Healthcare, is formed. |
| 1996 |
Valley Physicians Network, Inc. joins Lakeside HealthCare. This merger expands the overall organization’s ability to provide a broader network of health care services to the communities of Burbank and North Hollywood. |
| 1997 |
Lakeside HealthCare, Inc. is formed by combining the resources of Lakeside Health Services, Inc. and Medco Associates, Inc. Lakeside HealthCare provides management services to Lakeside Medical Group and Lakeside Medical Associates in addition to other IPAs and medical groups. The organization’s service area expands throughout the San Fernando and Santa Clarita Valleys. |
| 2004 |
Glendale Memorial Medical Group (IPA) joins with Lakeside Medical Group. As a result, Lakeside expands patient care into Glendale from its existing service areas in the San Fernando and Santa Clarita Valleys. |

Eastland Medical Group (IPA) and West Covina Medical Group join Lakeside HealthCare. Lakeside HealthCare’s network now includes the San Gabriel Valley. |
| 2007 |
Lakeside HealthCare, Inc., merges with Progressive Health Systems, an Agoura Hills-based physicians management company, and its Community Medical Group. The merger allows the company to expand contiguously, providing services to more than 250,000 patients from eastern Ventura County through the San Fernando and Santa Clarita Valleys to western San Gabriel Valley. |
| 2008 |
Lakeside HealthCare consolidates its corporate and management services organization under the name, Lakeside Systems, Inc. (LSI). LSI builds on its mutual business relationship with Providence Health & Services California Region (PH&S-CA) as the two form a strategic partnership. PH&S-CA, which operates four acute care medical centers in Los Angeles County, including Providence Saint Joseph Medical Center and Providence Holy Cross Medical Center. PH&S-CA makes a significant multi-million dollar equity investment in LSI, giving PH&S, CA a minority position in Lakeside. Under this agreement, Lakeside and PH&S-CA will dedicate resources to expand the integration and coordination of healthcare services in their similar quest to provide comprehensive, accessible and affordable healthcare to the communities they serve. |
