Performance Improvement Programs

At Lakeside Community Healthcare, we believe it is important to continually monitor the quality and effectiveness of our care and develop new and better methods of healthcare delivery. That is why we participate in a performance improvement program and quality care initiatives, which benefit both our physicians and our members.

Lakeside Community Healthcare physicians take pride in delivering quality healthcare to our patients. Our members are encouraged to directly engage in health decisions to promote wellness. The performance improvement program includes health services that health plans incorporate in their quality improvement programs. Some of these services include:

  • Preventive Medicine

    This facet of medicine deals with prevention of a disease rather than the treatment or cure. Our physicians encourage routine health screenings for our patients. Examples of these screenings include breast cancer screenings (mammograms), colorectal cancer screenings (colonoscopies) and cervical cancer screenings (PAP tests).

 

  • Disease Management

    This is the concept of improving quality of life for individuals with chronic conditions through integrated care. Our program includes careful monitoring of lab results and pharmacy data, adherence to the latest and most effective clinical guidelines for treatment and referrals to appropriate specialists as needed. Diseases treated through our disease management program include diabetes, coronary heart disease, asthma, heart failure, asthma, arthritis and depression. This proactive approach to healthcare benefits our patients, who can maintain or improve their health and quality of life.
